Accidents reported on area highways

I question what is it with these accidents seemingly occurring at the same places all the time? We have 18th street & I-70
57th street & I-70
Metropolitan & I-635
Is this a case of bad road design? Or is it incompetent drivers waiting until the last 100 feet to decide to change lanes and use an entrance/exit ramp? It’s almost a weekly thing at these three spots and weather seems to make no difference.
Swartz Rd. at n.b. 635 there is always that bump/expansion joint that creates problems in bad weather – seems some don’t know when to be careful there. 57th/ I 70 w.b is just flat out put the pedal to the metal just to get to the top of the next w.b. highway peak. 18th st. could be a problem of too much speed coming from the east, not enough lanes and slower traffic trying to speed up while others are exiting or just aren’t paying attention – it’s about being a defensive driver instead of just driving, excepting the Swartz area, imo.
